MJHS is seeking a Hospice Field Admission Nurse (RN) to perform initial physical and psychosocial assessments for potential hospice patients in private homes, hospitals, or nursing facilities. You will develop individualized care plans and admit eligible patients under NHPCO standards. Qualifications include an RN license (NYS) with a BSN preferred and 2+ years in med/surg/community health/oncology settings; NYS driver’s license is required. #J-18808-Ljbffr MJHS
