Key Responsibilities\n

• Conduct comprehensive assessments of patients’ mental health, considering emotional, behavioral, and social dynamics that may affect their care.

• Continuously monitor patient progress and adjust treatment plans as necessary in response to feedback from patients and other healthcare professionals.

Patient Population / Client Focus\n

This role focuses on patients in hospice care who are navigating complex emotional and psychological challenges associated with terminal diagnoses. You will work with individuals facing life-limiting illnesses, helping them and their families cope with grief, loss, and the process of dying. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
