Case Manager: Discharge Planning & Family Liaison

Vista del Mar Hospital seeks a case manager to join our multidisciplinary treatment team. You will act as a communication liaison between family and the treatment team, and you will develop and implement discharge plans including housing, transportation, and aftercare arrangements. Must be organized, able to work well with adolescent families, document proficiently, and maintain good phone etiquette.
