Social Worker (LSW/LCSW) - Home Health

Role Overview The Home Services Social Worker assesses the psychosocial needs of patients and families, providing resource support, short-term counseling, and crisis intervention in collaboration with interdisciplinary teams. Key Responsibilities Conduct psychosocial assessments, develop bereavement care plans, interpret cultural and spiritual beliefs, educate patients and families, and document interventions. Facilitate patient decision‑making, address end‑of‑life needs, and assist with advanced directives. Provide community resource referrals, help overcome barriers like transportation and access to essentials, and support applications for benefits such as Medicaid and SSDI. Respond to indicators of imminent death and ensure care aligns with hospice philosophy and Medicare regulations. Qualifications & Skills Master’s degree in Social Work with licensure as an LSW or LCSW in Illinois, and a valid driver’s license with proof of auto insurance.
