Work Environment\n

This position offers the convenience of a remote work environment, allowing for flexibility while delivering care from the comforts of your home. Regular communication with clients, families, and the interdisciplinary team will be facilitated through virtual meetings and collaboration tools. While your primary role will be remote, you may occasionally visit the detox center to participate in group therapy sessions and collaborate more closely with other professionals. Our center values adaptability; thus, scheduled working hours include a Monday to Friday structure from 8:00 AM to 4:30 PM, with potential evening and weekend flexibility to meet client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial conditions, evaluating emotional, mental, and social components impacting their recovery process.

• Collaborate with clients, their families, and healthcare professionals to develop and implement personalized care plans that address unique recovery goals.

• Provide empathetic counseling and emotional support, assisting clients in navigating their recovery journey and addressing challenging life transitions.

• Advocate for clients’ rights, recommending appropriate community resources and services tailored to their specific needs.

• Work closely with a team of nurses, addiction specialists, and other healthcare professionals to coordinate effective care strategies and monitor clients’ progress.

• Perform regular follow-up visits via telehealth platforms to assess clients’ well-being, review care plans, and adjust interventions as needed.

• Facilitate communication and mediation among family members involved in care to resolve issues and enhance support systems.

• Educate clients and families about community resources, support groups, and programs available, improving their access to social engagement opportunities.

• Maintain thorough and accurate documentation of client assessments, treatment plans, and communications, ensuring compliance with regulatory standards and ethical guidelines.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.
