Primary Care Physician

ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for this population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, while also championing a positive culture within the medical centers.

• PCP leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator
• Engages with transitional care team, case managers, and acute/transitional-care physicians for patients unable to come to the office
• PCPs are leaders in the organization and centers, expected to help champion a positive culture

Responsibilities

• Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
• Takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed care
• Engages with the hospitalist when one of their patients is in the hospital
• Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
• Leads their care team for patients able to come to the office
• Engages with the transitional care team and others for patients unable to come to the office (in hospital, SNF, LTC, or homebound)
• Has an active role in the management of their center and helps cover for other providers
• Takes an active role as needed in recruiting patients for the center and additional providers for the company

Compensation

• $221,141 - $315,915 Salary
• This position may also be eligible for bonuses or commissions
