RN Case Manager
Good Samaritan
Good Samaritan Health Center seeks an RN Case Manager to coordinate compassionate, whole-person care for underserved patients in our faith-based, community health setting. The RN Case Manager manages a caseload of medically and socially complex patients, conducts assessments, develops individualized care plans, and coordinates services across primary care, dental, behavioral health, and community resources. This role focuses on chronic disease management, health education, and addressing social determinants of health, working closely with an interdisciplinary team to advance health equity, quality outcomes, and access to affordable care.
