RN Case Manager

Good Samaritan Health Center seeks an RN Case Manager to coordinate holistic care for underserved patients in our faith-based, mission-driven clinic. The RN Case Manager performs comprehensive assessments, develops and manages care plans, and supports safe transitions across settings. Working closely with an interdisciplinary team, this role integrates medical, behavioral, and social needs, advocates for access to affordable services, and provides patient and family education. Ideal candidates bring strong clinical judgment, care coordination skills, cultural humility, and a passion for advancing health equity in a collaborative, community-focused environment.
