• Salary: $85,000-$95,000 per year (Based on years of experience) + Benefits
• Mileage Reimbursement: $0.65 per mile
• Cell Phone Reimbursement: $60/month

Position Overview

We are seeking a compassionate, experienced, and patient-focused Hospice Admissions Registered Nurse (RN) to join our team in Denton County, Texas.

As the first clinical point of contact for patients and their families beginning their hospice journey, the Hospice Admissions RN plays a vital role in providing compassionate care, conducting comprehensive clinical assessments, and ensuring a smooth and supportive admission experience.

The ideal candidate has hospice or senior care experience, excellent communication skills, strong clinical judgment, and the ability to work independently while collaborating effectively with an interdisciplinary care team.

-> Key Responsibilities

• Evaluate and assess patients to determine hospice eligibility.
• Conduct comprehensive nursing assessments for new admissions.
• Guide patients and families through the hospice admission process with compassion and professionalism.
• Develop individualized plans of care for newly admitted patients.
• Provide skilled nursing care based on individualized care plans.
• Collaborate with physicians and the interdisciplinary care team to ensure seamless patient transitions and continuity of care.
• Educate patients and families regarding hospice philosophy, services, and the plan of care.
• Maintain accurate, complete, and timely clinical documentation.
• Participate in a limited on-call rotation as required.
• Assist with additional clinical and administrative responsibilities as assigned.

-> Qualifications

• Current, active Texas Registered Nurse (RN) license .
• Minimum 1 year of hospice or senior care experience required .
• Strong understanding of hospice philosophy, eligibility requirements, and end-of-life care.
• Excellent communication, interpersonal, and patient/family education skills.
• Strong clinical assessment and documentation abilities.
• Ability to work independently and manage daily responsibilities efficiently.
• Flexible and adaptable to changing patient and family needs.
• Valid driver's license, reliable transportation, and current auto insurance.
• Must be at least 21 years of age.
• Successful completion of required background checks and professional reference verification.

-> Schedule

• Full-Time
• Monday–Friday, 8:00 AM–5:00 PM
• On-call rotation required
• Travel throughout the assigned territory required
