Compassionate Hospice RN Case Manager

Gentiva

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient assessments and deliver skilled nursing care in patients' homes or related settings. You will craft and adjust plans of care, coordinate with physicians, social workers, and the broader hospice team, and educate families on care goals and comfort strategies.
