Key Responsibilities\n

• Conduct in-depth assessments of seniors’ psychosocial needs, evaluating their emotional, mental, and social well-being to form a comprehensive understanding of each client’s situation.

• Develop and implement personalized care plans in collaboration with clients and their families, prioritizing their specific preferences and goals for care.

• Provide counseling and support to clients dealing with life transitions, grief, and chronic illnesses, encouraging resilience and fostering a sense of agency.

• Advocate for clients’ rights and help them access vital community resources, including support groups, financial assistance programs, and healthcare services.

• Work cooperatively with a multidisciplinary team, comprising nurses, caregivers, and other healthcare professionals, to ensure holistic care and seamless service delivery.

• Monitor clients' conditions through regular virtual visits, addressing their needs and modifying care plans as necessary to ensure optimal outcomes.

• Educate clients and their families on available support networks and community resources to promote social engagement and enhance quality of life.

• Maintain meticulous documentation of care plans, client interactions, and progress notes in the EHR system, ensuring compliance with ethical standards and organizational policies.

Qualifications\n

Compensation and Benefits\n

We offer competitive compensation ranging from $60,000 to $100,000 annually, commensurate with experience and qualifications.
