JOB REQUIREMENTS

Board: Interventional Cardiology board certification required
Licenses: Active PA license required
DEA: Yes, PA address required
Certifications: ACLS, BLS

SUBMISSION REQUIREMENTS

All required documentation must be submitted no later than 5 business days prior to start
Provider questionnaire required at time of submission
NPDB self-query required at time of presentation, within 6 months
Full disclosure on all malpractice required
If the role includes specific coverage dates, your submission must clearly state exact dates the provider can work
Submissions without rates will be rejected immediately
Required Submission Packet: CV, Coversheet, Questionnaire, NPDB (pulled within the last 6 months)

Duties & Responsibilities

Provide interventional cardiology clinical coverage
Perform diagnostic and interventional procedures including R/LHC and PCI
Provide 24-hour call coverage as scheduled
Collaborate with assigned clinical team and APP staff
Maintain accurate patient records within EPIC EMR
