Reporting to a Director of Population Health/Care Management with close partnership to Quality, Informatics, and Finance, the Nurse Manager Population Health will lead friendly nurses and a diverse team of RN care coordinators, social workers, community health workers, pharmacists, and analysts; responsibilities for the Population Health Nurse Manager include optimizing care pathways, reducing readmissions and total cost of care, advancing HEDIS and STAR measures, mentoring staff through evidence-based practice, strengthening shared governance, and leveraging real-time dashboards and clinical decision support to drive reliable daily management and high-reliability performance.
