We’re looking for a compassionate, organized, and dedicated NHTD/TBI Service Coordinator to join our team. In this role, you’ll be the go-to person for individuals applying for or participating in the NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) waiver programs. Your mission? Helping them navigate the system, access services, and build a more independent, integrated life.

From coordinating medical, social, vocational, and educational support to ensuring a smooth transition from nursing facilities back into the community, your work will directly impact lives in a powerful way.

What You’ll Do:

• Be the primary point of support for waiver applicants and participants
• Help individuals access Medicaid and other vital services
• Create, coordinate, and monitor person-centered service plans
• Assist individuals transitioning from nursing homes into the community
• Support those already living in the community to maintain independence and avoid unwanted placement
• Conduct field visits to evaluate and ensure participant is safe at home.

Education & Experience:

Bachelor’s degree with at least 1 years of relevant care plan or case management skills

• Reliable, supportive, and driven to make a difference
• Able to build trust and a strong rapport with participants
• Organized and deadline-oriented to keep participants on track with eligibility requirements
