Experience: Two (2) years of Med/Surg experience; Wound and Colostomy experience required.

License: Client RN License

Certifications: AHA BLS; current certification in one of the following: Certified Wound Ostomy Continence Nurse (CWOCN); Certified Wound Ostomy Nurse (CWON); Certified Wound Care Nurse (CWCN); Certified Wound Specialist (CWS); Wound Care Certified Certification (WCC).

Must-Have: MED/SURG NURSING - WOUND CARE - COLOSTOMY CARE - PATIENT ASSESSMENT - CARE PLANNING

Description: Conducts health assessment interviews focused on wound, ostomy, and continence related care. Performs focused physical assessments and develops plans of care based on diagnosis and individual client needs. Recommends medications and treatments as appropriate. Develops, revises, and evaluates standards of care. Assists in the selection of supplies, technology, and equipment. Assesses and documents patient responses to wound care, ostomy, continence, fistula, and complex wounds treatment. Evaluates the quality and delivery of care provided by clinical staff. Works collaboratively with the healthcare team and makes referrals to other disciplines as needed. Teaches, counsels, and assists individuals and families in taking responsibility for preventing complications related to ostomies, wounds, or continence problems. Develops, provides, and evaluates staff education programs, including new employee orientation and continuing education programs. Conducts comprehensive assessments of patients at risk for developing pressure injuries and initiates preventative measures. Escalates and communicates challenges identified, including ethical issues and conflict resolution. Collaborates with leadership to initiate and recommend best practices based on prudent assessment. Participates in unit and department quality improvement activities. Provides culturally competent patient, family, and staff education consistent with diverse belief systems. Maintains the security and confidentiality of data and information in accordance with patient rights policies. Achieves and maintains professional competency by engaging in relevant clinical practice activities. Identifies available community resources helpful to clients and assists them in utilizing such resources. Manages assets related to rented equipment, including specialty beds and negative pressure devices.
