Roles & Responsibilities:

Collaborate with nursing staff, residents, families, and other departments to provide individualized care.

Conduct rounds on each resident, assessing their condition and documenting findings per facility policy.

Notify nurses of any changes in residents' conditions or needs.

Assist with direct patient care, ensuring prompt response to call lights.

Maintain positive working relationships with staff, residents, families, and physicians.

Compliance / Notes:

Must be available to float to other units as necessary.

Availability for some weekends and holidays is required.
