Case Planner - Medical Foster Care - Manhattan, NY (Onsite) - Full Time -$68,000 Per Year Location: Manhattan, NY Position Type: Full Time Pay Range: $68,000 Per Year Description Join our team in a full-time role as a Case Planner specializing in Medical Foster Care. Work onsite in Manhattan, NY, collaborating with a dedicated team to support children with special medical needs and their families. This position requires strong organizational skills and flexibility. Responsibilities Work collaboratively with nurses and social workers. Provide intensive casework and fieldwork services. Train foster parents in specialized childcare skills. Conduct outreach to connect families with resources. Maintain detailed written case records. Partner with RNs for home visits. Perform additional duties as assigned. Qualifications Bachelor's Degree in Social Work or related field required. Experience in Foster Care preferred. Bilingual in Spanish/English preferred. Excellent verbal, writing, and organizational skills. Ability to work under pressure and meet deadlines. Proficient in computer literacy.
