Job Details Pay Range: $29.95 - $39.03 hourly Department: Health Department Job Type: Regular Represented Exemption Status: Non-Exempt Closing Date: July 02, 2026 Benefits Generous vacation and sick leave Exceptional medical and dental plans Pension Responsibilities Utilize electronic reports and review provider schedules to proactively assess needs for prevention screening and communication via EHR. Pro-actively perform pre-visit activities (e.g., scrubbing and huddling) for client visits as needed, and prepare patients for exams and procedures, including equipment set-up and specimen collection. Work with providers and teams to monitor chronic disease and prevention reports, initiating follow-up with clients or ensuring appropriate referrals as needed. Follow up on specialty referral reports, clinical treatments, prescriptions, medication cancellations, etc. Document clearly in the patient chart, using the SOAP method when indicated and EPIC "dot-phrases" and "smart phrases" as appropriate. Support back office clinic flow by providing direct provider support as needed, including rooming patients and ordering labs as directed. Provide after-visit instructions based on nursing experience and understanding of clients’ needs; make follow-up appointments if necessary. Administer medical treatments such as cleaning and dressing wounds, ear irrigations, suture removal, catheterizations, fluoride treatments, etc.; perform diagnostic tests such as labs, phlebotomy, EKG, etc. Work with RN and PCP to contribute to the plan of care. Complete documentation and charting as required by policy. Troubleshoot equipment issues and report necessary repairs as needed. Complete assigned quality‑control/assurance activities. Minimum Qualifications Completion of an Oregon State Board of Nursing approved program at the certificate level or higher and a license to practice as a Licensed Practical Nurse. Minimum one (1) year of nursing experience. Valid CPR/BLS certification upon hire. Preferred Qualifications Observational assessment of patients’ general health conditions. Knowledge of basic medication passing rules and protocols for patient safety, infection control, and Medication Aide certification limits. Clear oral and written communication skills. Experience working in a correctional health facility. Transferable skills relevant to the position. Other Requirements Background investigation required; employment contingent on a passing result.
