Behavioral Health Case Management Coordinator

Advocate Health's Behavioral Health Services seeks a qualified discharge planner to coordinate comprehensive discharge plans for child/adolescent units. You will work with families, physicians, and care teams to ensure timely, appropriate care and education about benefits and options. Must have a Master’s in Social Work or Psychology Counseling, licensure (LCSW/LCPC IL), and at least 3 years in behavioral health, with strong assessment, utilization review, and reimbursement knowledge. #J-18808-Ljbffr American Addiction Centers
