Job Summary:
To provide professional nursing care in accordance with the Nurse Practice Act, to collaborate with the physician's medical plan, and to execute the objectives and follow the policies of the hospital. To assist the surgeon in all procedures including advanced procedures in both the GI suite and the operating room.
Accountabilities:
- Assess, plan, implement, document, coordinate, and manage patient care according to department and hospital standards and giving consideration to age-specific accommodations.
- Utilizes the nursing process for all patients assigned to his/her care
- Implements nursing/physician orders that result in achievement of organizational, nursing and physician goals.
- Utilizes analytical skills to provide efficient patient care and be able to handle critical situations in a timely and safe manner.
- Able to provide care to the critical of patients including those requiring airway and/or ventilatory support and hemodynamic monitoring and intervention.
- Develops, implements and documents care plan based on nursing/physician orders, evidence-based practice and according to department standards.
- Demonstrates a comprehensive knowledge of normal physiology and disease pathophysiology, appropriate nursing and medical interventions and a knowledge of the principles of growth and development over the lifespan.
- Demonstrates appropriate skill in utilization of the EMR.
- Documents assessments, patient treatment, response and effectiveness of care according to department guidelines.
- Provides teaching and discharge planning based on assessment of self-care knowledge/ability on an ongoing basis.
- Ensures appropriate completion of chart and other documentation including transcription of physician orders, charge entry and medication reconciliation.
- Appropriately delegates all Associates under his/her supervision according to their skill level.
- Demonstrates initiative and self-direction in completion of assigned responsibilities in accordance with department needs.
- Performs selected therapeutic interventions and diagnostic procedures according to department standards as ordered to include but not limited to:
- Medication administration according to hospital policy
- Point of care testing
- Cardiac and hemodynamic monitoring
- Initiation and maintenance of oxygen therapy
- Specimen collection utilizing mobile lab when appropriate
- Phlebotomy (venipuncture)
- Adheres to Moderate Sedation Guidelines pre, intra and post procedurally.
- Thoroughly completes the GI Monitoring Record for all sedation cases.
- Successfully performs and records a Time Out before each procedure.
- GI Chart checks to be done daily, the day prior to the scheduled procedure.
- Assists patients physically by providing physical support through pushing, pulling, lifting, reaching, turning, kneeling, standing, sitting and balancing, and psychologically by providing emotional intervention.
- Demonstrates ability to perform with clinical proficiency in crises and medical emergencies.
- Demonstrates ability to be technically competent in all psychomotor interventions when administering physical and psychosocial care to patients.
- Implements decontamination, cleaning and reprocessing of endoscopic equipment and accessories according to policy and procedure. Utilizes appropriate methods (guided by standards) and techniques to achieve results in an effective and efficient manner. Completes logs
- Prepares and assembles all endoscopic instruments according to manufacturers recommendations. Effectively plans, organizes and completes preparation in the shortest, most efficient manner. Tests proper functioning of instruments.
- Prioritizes activities based on knowledge and comprehension of the schedule.
- Demonstrates knowledge and technical skills in use and care of endoscopic equipment and accessories.
- Assists the surgeon during all procedures including but not limited to EUS, EBUS, Diagnostic bronchoscopy, colon/rectal ultrasound, ERCP.
- Assists with administration of genetic testing in conjunction with the procedure.
- Demonstrates critical decision-making skills while providing care by analyzing data and modifying approach based on clinical status. Will consult with appropriate personnel and take appropriate action in a timely manner.
- Assists with specimen collection and proper labeling, preparation and transport.
- Assists with Ordering Supplies and checking expiration dates.
- Communicates and collaborates with patients, physicians, family members, and other health care disciplines in order to achieve excellence in patient care.
- Utilizes effective interpersonal skills in interactions with others.
- Involves patient/significant other in all aspects of the plan of care, serving as advocate when necessary.
- Consistently and continuously communicates, consults and shares pertinent information regarding changes in patient status and patient care.
- Demonstrates a willingness to accept and give constructive feedback for promoting professional excellence.
- Effectively communicates pertinent patient/significant other information during hand offs according to department standards.
- Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines and Joint Commission National Patient Safety Goals.
- Maintains safety precautions while providing patient care including but not limited to infection control isolation, patient/bed positioning, side rails, restraint utilization, skin care, patient/specimen transport, physical environment, fall prevention, seizure precautions, and work injury prevention.
- Analyzes data and responds to unusual/undesirable clinical status by consulting appropriate healthcare team members, including Rapid Response Team, and taking appropriate action in a timely manner.
- Maintains equipment in good working order, reports equipment failure, and initiates corrective action. When appropriate, adheres to defined computer generated quality monitoring or procedures for validation of equipment functionality.
- Safely administers medications and treatments as ordered according to hospital policy and formulary using the appropriate principles of safe medication administration, 2 patient identifiers.
- Safely handles various types of equipment, drugs and body fluids using knowledge of appropriate standard and cleaning precautions.
- Participates in the surgical time out prior to procedure.
- Follows staffing and on call guidelines. In the interest of patient care, responds to changes or needs within the Patient Services Team by cooperating with staff rotation, reassignment to other departments and weekend/holiday scheduling.
- Actively participates in and supports departmental and organizational quality initiatives and goals.
- Attends and participates in meetings according to department standards.
- Participates in unit/hospital committees
- Participates in hospital and unit quality and safety initiatives
- Assumes additional duties and responsibilities as assigned.
- Demonstrates and maintains competency in the care of patients during all procedures including proper handling, use and cleaning of all equipment
- Demonstrates and maintains technological competency in skills required for participation in advanced procedures such as Endoscopic Ultrasound (EUS), Endoscopic Bronchial Ultrasound (EBUS), Diagnostic Bronchoscopy, Colon/Rectal Ultrasound, Endoscopic Retrograde Cholangial-pancreatography (ERCP), genetic testing, and GI procedures. This includes assistance with fine needle aspiration, slide preparation and ordering for laboratory specimens, stent placements and removal, Spy procedures.
- Demonstrates and maintains technological competency in basic procedures such as colonoscopy, EGD and bronchoscopy.
- Demonstrates competency to perform procedures performed in operating room and GI suite.
- Demonstrates and maintains proficiency in IV administration.
- Demonstrates the ability to safely handle, clean and use all equipment
- Recognizes legal responsibilities and functions within limitations as specified by the Pennsylvania Nurse Practice Act.
- Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
- Other duties as assigned to support the unit, department, entity, and health system organization
