ABOUT THIS TEMP POSITION

This is a temporary, full‑time (40 hours/week) assignment expected from July 2026 through December 2026, subject to change based on business need. If selected, you would be an employee of a temporary employment agency that we would connect you with.

WHAT YOU’LL BE RESPONSIBLE FOR

Develop and manage individualized intervention plans for Medicare Dual Eligible Special Needs Plan (D‑SNP) members, promoting optimal, cost‑effective outcomes. Collaborate with the interdisciplinary Care Coordination team to facilitate quality outcomes across the continuum, educating members, caregivers, providers, and external agencies. Participate in program evaluation and quality improvement efforts to ensure D‑SNP program effectiveness and regulatory compliance.

WHAT YOU’LL NEED TO BE SUCCESSFUL

Knowledge of: Case management and coordination principles, including assessment, care planning, discharge planning, and documentation. Utilizing community resources for special services. Reporting methods for abuse or neglect cases. Community and behavioral health systems. Needs of older adults, individuals with disabilities, and those with complex medical or psychosocial needs. Ability to: Interview members effectively to determine strengths, problems, prognosis, functional status, goals, and service needs, establishing short‑term and long‑term goals. Assess psychosocial needs and conduct culturally competent interviews. Use proactive customer‑service skills in responding to complex and demanding situations. Advocate on behalf of members to assure quality of care and attainment of appropriate goals. Education and Experience: Possession and ongoing maintenance of a Licensed Clinical Social Worker (LCSW) license issued by the State of California. Master’s Degree in Social Work or related field and at least two years of social service experience in an acute care, community‑based, or managed care environment, including development and management of member‑centered care plans; or an equivalent combination of education and experience.

OTHER INFORMATION

We operate in a hybrid work environment. Interviews will occur remotely via Microsoft Teams. Attendance at quarterly company‑wide events or department meetings is expected. In‑office or in‑community presence may be required for some positions, depending on business needs. This temporary assignment does not provide Alliance benefits. Temporary employees will be connected to a staffing agency with separate benefit options.

COMPENSATION INFORMATION

Zone 1 Pay Range: $55.00 – $58.00 per hour. Typical areas include Santa Cruz, San Benito, Monterey Counties, Bay Area, Sacramento, Los Angeles, and San Diego. Zone 2 Pay Range: $52.00 – $55.00 per hour. Typical areas include Mariposa and Merced Counties, Fresno area, Bakersfield, Eastern California, San Luis Obispo area, and the Central Valley (except Sacramento). The applicable salary ranges are based on work location and may change in the future. Compensation will be determined by our philosophy, analysis of qualifications, experience, education, internal equity, market factors, and geographic location.
