Hiring: Remote Social Worker Case Manager – Healthcare Work Arrangement: First 2 days on site for training; fully remote after training Job Summary We are looking for an experienced Social Worker Case Manager to support care coordination and case management activities for members. The ideal candidate will have strong experience working with patients, physicians, healthcare providers, and external agencies to coordinate quality care and ensure appropriate services are delivered across the continuum. This role will focus on care planning, discharge planning, utilization coordination, referrals, authorizations, psychosocial assessments, and connecting members with appropriate healthcare and community resources. Key Responsibilities Plan, develop, assess, and evaluate care provided to members. Partner with primary care and specialist physicians to complete medical and psychosocial assessments and develop individualized care plans. Coordinate interdisciplinary care, including utilization management, transfer coordination, and discharge planning . Make referrals to appropriate community resources, healthcare services, and external providers . Monitor the level and quality of care provided to members. Identify opportunities to improve quality of care, resource utilization, and cost effectiveness . Required Qualifications Experience in Social Work, Case Management, Care Management, or a related healthcare environment . Strong knowledge of healthcare case management and care coordination processes. Experience with discharge planning, referrals, authorizations, and transitions of care. Ability to collaborate effectively with physicians, nurses, healthcare providers, patients, families, and external agencies. Strong understanding of healthcare benefits, policies, procedures, and regulatory requirements. Excellent communication, organization, documentation, and problem-solving skills.
