Sutton Park Center is seeking to hire a Director of Social Work with Sub Acute and Long-Term Care experience to join our dynamic team of professionals. Implement History and Assessment, Advanced Directives and End of Life Planning. Audit psychology paperwork, hospital discharges, room changes, PASRR, and Advanced Directives. Completing all required documentation in the medical record (EMR). Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills. Demonstrates judgment of safety and confidentiality issues. Strong verbal and written communication skills. Strong customer service focus required. Knowledge of the community resources. Working Knowledge of Sigmacare. Education & Experience LMSW Required. Good working knowledge of long term care regulations including documentation requirements. Prior experience in a Skilled Nursing Facility or Long-Term Care setting preferred. Job Type: Full-time Salary: $85,000 per year Schedule: M-F, Morning shift Work Location: In person #J-18808-Ljbffr
