Key Responsibilities\n

• Deliver counseling, therapy, and psychotherapy to clients, tailoring interventions to their individual needs and circumstances.

• Prepare comprehensive treatment plans and effective discharge plans to ensure a continuum of care for your clients.
\n
• Conduct thorough assessments by gathering data through interviews, case histories, and observational techniques to inform your therapeutic strategies.
\n
• Analyze data to identify root causes of mental health issues and collaborate to determine appropriate referrals to external specialists when needed.
\n
• Maintain a manageable caseload consisting of both individual and collateral therapy sessions, fostering a supportive therapeutic relationship with clients.

• Ensure compliance with internal policies and external regulations by meticulously reviewing client case files for quality and accuracy, including timely completion of required documentation such as assessments, care plans, and progress notes.

• Adapt and train others on assessment protocols in accordance with industry best practices and health plan guidelines.

Patient Population / Client Focus\n

In this role, you will primarily support clients dealing with substantial mental health challenges often associated with the hospice setting. This includes managing chronic pain, grief, and anxiety, as well as providing mental health support to both clients and their families during end-of-life care. Your ability to connect and empathize will play a pivotal role in enhancing the clients' emotional well-being.

Qualifications\n

• A Master's Degree in Social Work from an accredited graduate program.

• A valid and active California Clinical Social Worker License (LCSW) with no restrictions.

• A minimum of 2 years of experience working in a healthcare setting, with at least one year in a mental health environment.

• Demonstrated clinical experience in a social service or healthcare context, including case management and therapeutic intervention.

• Strong conflict management skills and the ability to navigate complex interpersonal dynamics.

Preferred Experience\n

• Experience working within a hospice or palliative care setting is highly desirable.

Compensation and Benefits\n
