Licensed Clinical Social Worker

Visiting Nurse & Hospice of Litchfield County is in search of a Licensed Clinical Social Worker (LCSW) to join our team covering the Torrington & Greater Farmington Valley area. We provide Hospice and Home Care services to the residents of Litchfield County and Farmington Valley with a talented and experienced clinical team. The social worker as a member of the care team assists in long‑term planning, provides education and assists patient and family members to access community resources to facilitate home safety. The social worker is a key member of our hospice and palliative care teams and must be comfortable with end‑of‑life care. Education and/or Experience Required: Master’s degree in social work from an accredited graduate program Experience in home care and hospice preferred License and/or Certifications Required: Licensure by CT State Department of Public Health as a Clinical Social Worker Per Diem position Send resume to View email address on click.appcast.io #J-18808-Ljbffr
