Outpatient Physical Therapist – Full Time – Keller Oaks Healthcare Center, Keller, TX Keller Oaks Healthcare Center is a post‑acute care facility offering long‑term, short‑term, and geriatric outpatient services. We seek a dedicated Outpatient Physical Therapist for our mobile Skilled Nursing rehabilitation program. This position offers paid travel time. Responsibilities Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans that adhere to regulatory and clinical standards. Deliver rehabilitative, skilled, and medically necessary treatment interventions in accordance with regulatory and clinical practice requirements. Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care. Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders. Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements. Qualifications Physical Therapist license is required. Travel and reliable transportation required. Open to Physical Therapists at all experience levels; new graduates are welcome. Passion for learning, accountability, and a dedication to patient care.
