As a Nurse Practitioner (NP) at CenterWell Senior Primary Care, you will be part of a collaborative team. This team is diverse and includes physicians, pharmacists, care coaches, behavioral health specialists, and more. All of these professionals work together to deliver personalized, high-quality care.

Your Role & Responsibilities

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will manage referrals, hospital/SNF coordination, DME, and home health services.
• You will document care with support from quality-based coders.
• You will participate in on-call rotation and support growth plans at your center.
• You will have experience providing value-based care.

What We're Looking For

• A Bachelor's degree
• Masters in Science of Nursing /PA
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state
• Must be passionate about contributing to an organization focused on improving consumer experiences
• Must be willing to participate in limited on-call coverage by phone
• You will report to the Associate Medical Director in market.
• This role is onsite in our Richmond Road clinic.

Scheduled Weekly Hours: 40

Pay Range: $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
