Weekend RN Care Manager: Care Coordination & Discharge

Piedmont Mountainside Hospital is seeking a Care Manager to coordinate patient care, oversee progression and discharge planning, and ensure timely and safe discharges for all patients in their caseload. This role supports a collaborative healthcare team within a hospital setting. The position requires an RN license in Georgia (or multi-state licensure) and at least 2 years of acute/post-acute experience, with InterQual/Milliman experience preferred.
