Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring, under the supervision of a licensed nurse

• Develop and implement individualized care plans in collaboration with interdisciplinary team members

• Perform routine vital sign checks, identify changes in patient condition, and report to the nursing team

• Maintain accurate and detailed records of patient care, treatment, and progress

• Collaborate with caregivers, therapy staff, and other healthcare professionals to ensure high-quality care

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Evening Shift LPN certification required

• Current licensure in the state of Pennsylvania

• Ability to work variable shifts, including evenings and weekends

• Strong communication and interpersonal skills

• Physical stamina to perform duties in a fast-paced environment
