Registered Nurse - LTC/SNF

Registered Nurse

Provides comprehensive nursing care to patients in long-term care or skilled nursing facilities, ensuring safety, comfort, and adherence to care plans. Collaborates with healthcare teams to deliver high-quality, patient-centered services.

Responsibilities:

• Assess patients health conditions and establish personalized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor vital signs and observe for any changes in patient status.
• Document patient information accurately and update medical records.
• Educate patients and families on health management and care procedures.
• Maintain compliance with facility policies and legal requirements.

Qualifications:

• Valid RN license in the state of practice.
• Minimum of 1 year experience in LTC/SNF settings preferred.
• Ability to prioritize and manage multiple care responsibilities.
• Strong communication and interpersonal skills.
• Current CPR and BLS certifications.
• Must bring a valid government-issued ID and wear a photo badge at all times.
