Ambulatory Care Clinical Pharmacist - Academic Offices in Pontiac and Livonia

Employment Type: Full time Shift: Day Shift Description: Pharmacist will be working at offices in Pontiac and Livonia Trinity Health IHA Medical Group is seeking a compassionate and patient-focused Ambulatory Care Clinical Pharmacist to join our outpatient care team. In this role, you will Provides ambulatory clinical pharmacy services (including disease management and comprehensive medication review and Population Health Management under a collaborative practice agreement at multiple sites within the organization. What You'll Do: Provides pharmaceutical care by participating in patient care during outpatient clinic visits. Performs Population Health initiatives. Accurately documents interventions and outcomes in drug therapy quality improvement efforts. Collaborates with other clinicians as part of the care team as well as other pharmacists. What We're Looking For: EDUCATION: Doctor of Pharmacy/Pharmacology degree required. Completion of self-management support training preferred. CREDENTIALS/LICENSURE: Valid, unrestricted license as a Pharmacist in the State of Michigan. Valid CPR certification. Valid unrestricted State of Michigan Controlled Substance license. MINIMUM EXPERIENCE: Five (5) years of relevant experience is preferred. A Post-Graduate Year 1 (PGY1) Pharmacy Practice Residency or Post-Graduate Year 2(PGY2) Specialty Residency can substitute for relevant experience. Board Certified Ambulatory Care Pharmacist (BCACP) preferred and BCACP Board-Eligible considered. Why Join Trinity Health IHA Medical Group?
