Job Title Inpatient Transition Clinical Care Partner – ED Position Summary The Emergency Department Inpatient Clinical Care Partner (CCP) collaborates with the RN to provide patient-specific observation, care, and intervention for at‑risk patients in the Emergency Department and Inpatient Psychiatric Departments. The CCP assists with activities of daily living, monitors vital signs, and works with a diverse and vulnerable population to reduce and prevent self‑harm. Key Responsibilities Perform observation, care, and intervention for patients as directed by unit protocol. Assist with activities of daily living: feeding, bathing, ambulating, and vital sign assessment. Communicate effectively with patients, staff, and other disciplines in a constructive, professional, and therapeutic manner. Collaborate with the patient care team in the formulation and application of the patient plan of care. Support related clinical environments and clinical support functions as required. Minimum Qualifications Three (3) years of related experience providing patient care and observation; or an equivalent combination of education and experience. One (1) year or greater experience working with vulnerable and behavioral high‑risk populations, specifically the cognitively impaired or those suffering from psychiatric disorders. BART or BLS at time of hire with a commitment to obtain BART within six (6) months. Commitment to complete UCSD CPI/behavioral management/de‑escalation training within 6 months. Demonstrated empathy and the ability to understand others’ perspectives even in challenging situations. Excellent verbal and written communication skills, including active listening and clear expression. Preferred Qualifications Prior experience as a Certified Nursing Assistant (CNA). Experience caring for cognitively impaired, acute and chronic psychiatric illness, and/or substance abuse comorbidities. CPI training and/or de‑escalation experience. Bilingual proficiency in Spanish/English. Special Conditions Flexible to work variable hours and locations including overtime, nights, weekends and holidays. Employment subject to a criminal background check and pre‑employment physical. Initial and annual training requirements subject to modification by the Emergency Department. New hires who fail to complete initial training requirements will be released from employment within the probationary period. Compensation Annual Full Pay Range: $57,399 – $71,347 (will be prorated if the appointment percentage is less than 100%).
