Hospice Social Worker, LMSWFull TimeThis position will include field visits in Queens and Nassau counties, in addition to in-patient visits.The Parker Jewish Institute for Health Care and Rehabilitation is seeking a full time Social Worker to join the team in the Hospice department. Parker Jewish Institute Hospice offers the finest in hospice services from an interdisciplinary team of health care professionals. Position OverviewReporting to the Hospice Administrator, the Hospice Social Worker delivers appropriate social work services to assigned patients, residents, family members and significant others.
