Foster Care Case Manager

Summary The Foster Care Case Manager performs duties related to the Foster Care Ministry, enabling foster families and children to function at the highest level of competency in carrying out the ministry’s and agency’s policies, procedures, and programs. Responsibilities include teamwork, group work, working with families according to developed plans, family and custodian contacts, community resource coordination, and case record management. Job Title Foster Care Case Manager Department Foster Care Reports To Foster Care Supervisor FLSA Status Exempt Qualifications Minimum age: 23 years. Bachelor’s degree in Human Services or related field from an accredited university or seminar. Two years of experience working with the population served. Valid driver’s license and ability to operate an agency vehicle to transport children. Capability to manage, plan, organize, implement, monitor, and maintain agency programs while adhering to accreditation standards and N.C. licensing requirements. Meets general qualifications for employment as outlined in the institution’s personnel policies. Respects the cultural diversity of children and strives to create a positive relationship. Essential Duties and Responsibilities Prepare for and participate in regular supervisory conferences. Participate in 90‑day and annual staff performance evaluations. Keep supervisor informed on current status of total work responsibilities. Policies and Procedures Operate within guidelines of the job description and the Program of Intentionalized Services. Represent the agency’s philosophy, purpose, and programs to groups, individuals, and referral agencies professionally and objectively. Professionalism/Teamwork Demonstrate knowledge of and ability to work as a member of the foster home team. Support roles of other team members and respect their right to be involved with the child and family. Share necessary information with team members for foster home and treatment team decisions. Support the team in providing daily care and maintaining a positive environment. Participate in team meetings and other staff meetings. Monitor effectiveness of the team’s functioning. Conduct foster parent training on appropriate subjects as assigned. Provide leadership in creating and maintaining therapeutic milieu. Work closely with the Division of Social Services Social Worker. Respect cultural and socioeconomic diversity of staff and children. Participate in the agency’s Performance Quality Improvement program as needed. Clients / Foster Families Facilitate FAC, admission, intake, transfer, and discharge conferences. Provide leadership in child and family conferences and coordinate and implement the Plan of Service and Plan of Service Reviews with each family. Recruit and license foster families, provide required training. Offer support, encouragement, recognition of feelings and problem‑solving that complement family work and therapy. Arrange family contacts, emergency contacts, financial accounting, scheduling of appointments, behavior infractions, consents, in‑home plans, and reporting medical, school, and other daily events. Establish and maintain relationships with resources for client systems and assist family/custodian in securing appropriate internal and external community resources. Conduct psycho‑educational groups covering topics such as peer pressure, social skills, anger management, independent living, group building, communication, decision making, chemical dependency, self‑esteem. Support medical, dental, and other appointments for youth in care. Clarify the organization’s “Family Systems” approach to family work and foster reunification goals. Conduct routine home visits. Recruit foster families by presenting the need to individuals and groups. Conduct foster home application studies in accordance with policies, procedures, and licensing standards. Submit all required paperwork for foster home initial licensing and renewals. Orient and train new foster parents to understand their roles, responsibilities, and expectations. Provide ongoing training to meet state and accreditation requirements and special needs of foster parents. Be available to foster parents on a regular or emergency basis to assist with identified issues and crisis needs. Assist foster parents with securing community resources identified by the team. Record Keeping Maintain all client files and documentation of case activities current, including Admission Documents, MIGS, plans of service, plan reviews, summary dictations, crisis management plans, outcome measurements, consents, progress notes, transfer and discharge memos, alert slips, court reports, after‑care plans. Knowledge and Training Continuously increase knowledge and skills by staying current with trends in foster care. Consistently apply professional knowledge and skills in fulfilling job responsibilities. Maintain a body of knowledge and skills necessary for the position. Demonstrate knowledge of current service trends for families and children. Develop a pool of external resources for supporting services as needed. Show working knowledge of skills in working with family systems, individuals, and groups. Participate in Family Systems trainings. Equipment Operate standard office machines (copier, computer, fax, calculator) and use Microsoft Outlook, Excel, and Word. Language Skills Read, write, and complete necessary documentation. Effectively present information and respond to questions from clients, customers, resources, and the public. Mental, Visual, Auditory Demand Interpret a variety of instructions in written, oral, diagram or schedule form and draw conclusions. Contact With Others Maintain regular contact with colleagues in all departments and with outside individuals and groups. Represent the organization positively. Proper handling of outside contacts is crucial for the organization’s image. Confidential Data Comply with agency policies and procedures concerning confidentiality and HIPAA regulations for all client information, including day‑to‑day care, case records, family information, medical information, and psychological services. Physical Demands Necessary to sit, talk, hear, occasionally stand, walk, and reach with hands and arms. Vision required includes close vision and ability to adjust focus. Reasonable accommodations will be made for disabilities. Work Environment Usual office working conditions.
