LPN - LTC

LPN - Full Time, 40 Hours Weekly

The Licensed Practical Nurse (LPN) will perform direct and indirect resident care under the direction of licensed nursing personnel. This role involves meeting the diverse needs of adult and geriatric patients while ensuring adherence to all policies and procedures. The LPN must demonstrate the appropriate knowledge and skills necessary to provide care relevant to the age and specific needs of the resident population.

Responsibilities

• Ensure all resident activities of daily living (ADLs) are met as per the care plan, maintaining a safe and comfortable environment.
• Maintain resident environment and safety by promptly responding to call lights, managing safety equipment, and following behavior and fall prevention protocols.
• Assist with meeting resident nutritional needs, including meal delivery and feeding assistance while honoring residents' preferences.
• Provide rehabilitative care as outlined in the resident care plan, facilitating mobility and use of assistive devices.
• Report resident rights, refusals, and any changes in physical, emotional, or psychosocial status to the Charge Nurse in a timely manner.
• Comply with safety protocols regarding potential exposure to blood-borne pathogens during care tasks.

Required Experience / Certifications / Licensure

• New York State LPN Licensure
• BLS Certification (AHA or RQI only)
• Minimum of one (1) year of experience as an LPN, preferably in Long-Term Care (LTC) or a similar capacity
• High School Diploma or GED

Schedule

This position requires a full-time commitment of 40 hours per week, with a specific requirement to work through the holiday season. Candidates must be available to start on a Tuesday or Thursday.
