Targeted Case Manager I - Mental Health & Recovery Support
Honor Community Health
Honor Community Health is seeking a Full Time Exempt, Level 1 Targeted Case Manager to support individuals in sustaining mental health and substance use recovery. The role reports to the Director of Care Management, with standard hours Monday–Friday 8:30am–5:00pm. Qualifications include a bachelor’s degree in social work or a related field with licensure as an RN, plus at least one year of direct service experience in behavioral health.
