Responsibilities Provide administrative and operational support to the Care Management Department. Assist with patient throughput activities and discharge coordination. Support regulatory and documentation requirements. Communicate with clinical teams, community resources, and insurance providers. Ensure patients receive information regarding post‑discharge services, rights, and options. Maintain accurate records using electronic medical record (EMR) systems. Utilize office equipment and Microsoft Office applications effectively. Qualifications High School Diploma (preferred).
