Key Responsibilities\n

• Provide guidance and therapeutic interventions that promote emotional, physical, and mental growth in patients, utilizing established Agency and Federally Qualified Health Center (FQHC) guidelines.

• Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations, formulating insightful treatment goals to present to the primary care provider.

• Deliver brief, focused therapeutic interventions, ensuring prompt support for patients in need of mental health services.
\n
• Advise primary care providers regarding optimal treatment settings for patients, determining whether to manage care within our facility or refer them to specialized mental health resources.

• Monitor and follow up on patient progress, making necessary adjustments to treatment plans as needed to enhance patient outcomes.
\n
• Create relapse prevention plans and strategies that assist patients in maintaining stable functioning.
\n
• Detect and develop intervention plans for “at risk” patients, collaborating with colleagues to prevent deterioration of their conditions.
\n
• Document patient progress, ensuring that all diagnostic information is accurately recorded within treatment charts while keeping primary care providers informed of patient developments.

• Provide educational support to patients, facilitating their involvement in treatment decisions and encouraging healthy lifestyle changes.

• Consult with clinic management and interdisciplinary team members about enhancements to behavioral services and performance evaluation measures.

• Participate in ongoing professional development and training to remain abreast of the latest therapeutic techniques and community resources.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
