Experienced Intake Coordinator

We are seeking an experienced Intake Coordinator who can confidently manage the intake process, provide excellent customer service, and ensure a smooth and professional experience for every client.

KEY REQUIREMENTS

• Fluent in English is REQUIRED — strong verbal and written communication skills are essential for this position.

• Spanish-speaking is PREFERRED — candidates who can communicate effectively with Spanish-speaking clients and families are highly encouraged to apply.

• Previous Intake or Healthcare Coordination experience is REQUIRED.

We are looking for someone who can step into the role with confidence and understands the importance of timely follow-up, accurate documentation, organization, and exceptional customer service.

What You'll Do

• Serve as a primary point of contact for new clients, families, referral sources, and healthcare professionals.

• Conduct initial intake calls and gather accurate client information.

• Communicate with clients and families regarding services, intake procedures, and next steps.

• Communicate with Spanish-speaking clients and families when applicable .

• Explain available services and guide clients through the intake process.

• Coordinate assessments, appointments, interviews, and admissions.

• Collect, review, and track required documentation.

• Follow up with clients and referral sources regarding pending information and requirements.

• Maintain accurate and organized client records.

• Coordinate with internal teams to ensure a smooth transition from referral to service.

• Monitor pending referrals and ensure timely follow-up.

• Handle phone calls, emails, SMS, and other forms of communication professionally.

• Address client and family questions or concerns and escalate issues when appropriate.

• Maintain confidentiality and follow agency policies and applicable healthcare regulations.

• Provide a welcoming, professional, and compassionate experience to every client from the first interaction.

What We're Looking ForREQUIRED

• Previous experience in intake, healthcare coordination, home care, admissions, case management, or a related healthcare position.

• Fluent in English.

• Excellent verbal and written communication skills.

• Strong organizational and time-management skills.

• Excellent follow-up and attention to detail.

• Comfortable handling multiple calls, referrals, and priorities.

• Professional, dependable, and compassionate.

• Strong computer and documentation skills.

PREFERRED

• Spanish-speaking skills — highly preferred.

• Experience working in home care, healthcare, behavioral health, or services for individuals with disabilities .

• Experience with intake or electronic healthcare/client management systems.

• Knowledge of healthcare documentation and referral processes.
