Community Outreach Specialist

Job Summary

A mission-driven nonprofit organization in Santa Clara, CA is seeking a Community Outreach Specialist for a direct hire opportunity supporting Enhanced Care Management services. This role is ideal for a compassionate and organized professional with 3 to 5 years of experience in community outreach, public health, care coordination, social services, or nonprofit program support.

The Community Outreach Specialist will help adults, caregivers, and families navigate complex health and social service needs by conducting outreach, supporting intake and enrollment, coordinating referrals, and connecting participants with valuable community resources. Key Responsibilities

- Perform telephonic and in-person outreach to engage potential Enhanced Care Management participants and explain available program benefits.
- Meet with participants in community, office, and other approved settings to support intake, enrollment, consent collection, and ongoing engagement.
- Use research tools, provider contacts, referral lists, and health plan information to locate and follow up with eligible participants.
- Coordinate referrals to community-based, social support, and health-related resources based on participant needs.
- Maintain accurate documentation, data entry, reporting, and compliance records in accordance with confidentiality and program requirements.
- Participate in community outreach events, presentations, audits, and additional program support activities as assigned.

Compensation and Benefits

- Direct hire opportunity.
- Pay range: $28.00 to $30.00 per hour.
- Hybrid schedule with 4 days in office and 1 day work from home once training is complete.
- Planned local travel during working hours as needed for in-person outreach and community events.

Required Qualifications and Skills

- Conduct outreach and engagement for adults, caregivers, and families eligible for Enhanced Care Management services.
- Support participant intake, enrollment, consent collection, referrals, and follow-up activities.
- Maintain accurate participant records, documentation, data entry, and program compliance reporting.
- Represent the program at community meetings, outreach events, and approved participant locations.

- 3 to 5 years of relevant experience in community outreach, public health, social services, healthcare administration, nonprofit services, or care coordination.
- Strong ability to communicate clearly and compassionately with individuals from diverse backgrounds.
- Comfortable conducting outreach by phone, in person, and in community-based settings.
- Strong organizational skills with the ability to manage follow-up tasks, documentation, referrals, and deadlines.
- Ability to maintain confidentiality and follow program, compliance, and documentation requirements.
- Proficiency with basic computer systems, data entry, email, and electronic records or case management tools.
- Reliable, professional, and service-focused with strong problem-solving and relationship-building skills.
- Ability to travel locally during working hours for outreach visits, participant engagement, and community events as needed.

Preferred Qualifications

- Experience with Enhanced Care Management, Medi-Cal programs, health plans, or community-based care coordination.
- Prior nonprofit, public health, behavioral health, aging services, disability services, or human services experience.
- Familiarity with Santa Clara County community resources, social support programs, and referral networks.
- Bilingual or multilingual communication skills are helpful for serving diverse community populations.
