What You'll Do

• Provide physical therapy services to outpatient, inpatient, and occasionally SNF or home health patients
• Collaborate with multidisciplinary teams to develop and implement patient treatment plans
• Document patient progress accurately using Epic EMR system
• Support community health initiatives, including local schools
• Maintain compliance with all licensing and safety standards
• Rotate weekend and rare holiday shifts as scheduled

What You Need

Required:

• MN Physical Therapist license
• BLS certification (within 12 months of hire)
• 1+ year clinical experience in a healthcare setting
• Experience with electronic medical records, preferably Epic

Preferred:

• Outpatient or inpatient experience
• Strong communication and teamwork skills
• Ability to adapt to various care settings
