Responsibilities include psychosocial evaluation, crisis intervention, psychotherapeutic support and counseling, and/or psychotherapy to patients and their families. Act as an advocate, assisting assigned patients in physical, emotional, and social adjustment, and facilitate individualized care plans within a multidisciplinary team. Maintain continuity of care by developing and implementing patient discharge plans from admission onward, collaborating with patients, families, guardians, caseworkers, and community resources. Provide ongoing monitoring, individual and group counseling, and facilitate adjustment to secondary effects of illness. Serve as liaison between facility, family, government, and community agencies, document interventions, complete required paperwork, and ensure smooth transitions of care. Respond to crisis management situations per hospital and departmental policy. Perform additional duties as assigned. Responsibilities Perform psychosocial and psychiatric evaluations of patients with varying diagnoses. Conduct crisis intervention and provide therapeutic support. Develop individual care and discharge plans in collaboration with multidisciplinary teams. Facilitate transitions and coordinate referrals to community resources. Document social work interventions and complete required paperwork. Respond to crises in accordance with hospital policy. Qualifications Master’s degree in Social Work. Current license as a Licensed Master’s Social Worker in Michigan or a limited license to engage in practice at the master's level, with full licensure within three years of hire. One to two years of experience in psychiatric evaluation, disposition, and treatment with patients exhibiting psychiatric, substance abuse, or physical impairments. Skills Required Ability to work with patients with difficult medical and emotional problems. Strong interpersonal skills for productive relationships with patients, families, community agencies, and hospital staff. Effective written and verbal communication with diverse populations. Analytical ability to assess medical/psychotherapeutic circumstances and determine needed interventions or referrals. Good visual acuity and physical ability to move throughout the hospital. Capacity to cope with stressful situations, manage multiple priorities, and handle complex psychosocial issues.
