Develop, implement, and continuously evolve patient‑centered care plans. Perform ongoing nursing assessments and adjust care plans in real time based on changing conditions. Administer treatments, medications, and interventions as prescribed—bringing quality care to the home. Empower patients and families through health education and hands‑on guidance. Support proactive discharge planning to ensure seamless care transitions. Serve as Case Manager when designated, coordinating holistic care and ensuring continuity. Maintain clear, timely, and accurate clinical documentation. Collaborate closely with physicians, care coordinators, and interdisciplinary teams. Serve as a vital liaison between the patient, family, and healthcare providers. Participate in case conferences and advocate for optimal patient outcomes Additional Duties Participate in on‑call rotation for 24/7 care continuity. Coordinate necessary equipment, services, and resources for effective home treatment. Supervise and evaluate home health aides every 14 days, ensuring top‑tier care delivery. Qualifications For a Registered Nurse (RN) Graduate of an accredited nursing program (Associate or BSN preferred). Active RN license in the state, current CPR certification, and valid driver’s license. 1–2 years of acute care experience Home health care experience preferred. Strong communication, critical thinking, and clinical documentation skills. Capable of lifting, positioning, and transferring patients—hands‑on and ready for action.
