Hospice Care Planner — RN Admissions & Care
MJHS
MJHS Hospice & Palliative Care offers comprehensive services in inpatient facilities and community settings. The Hospice Care Planner will evaluate referrals, establish care plans, and facilitate admission from hospital discharge to an appropriate hospice program. As a key member of the business development and marketing team, you will build relationships with physicians, social workers, case managers and discharge planners to coordinate patient care and family support. #J-18808-Ljbffr MJHS
