Key Responsibilities\n

• Conduct thorough intake and psychosocial assessments to gather comprehensive client information and understand their emotional, social, and psychological needs.

• Develop and present individualized treatment plans to the clinical team, ensuring recommendations are tailored to meet the specific needs and goals of each client.

• Participate actively in clinical case conferences, bringing your insights to the table to enhance team collaboration and client outcomes.

• Deliver educational lectures focused on chemical dependency, ensuring clients and their families are informed about treatment options and coping strategies.

• Provide individual and group counseling services centered on chemical dependency, utilizing evidence-based approaches to foster recovery and resilience.

• Facilitate case management services, coordinating resources and referrals for clients to ensure they receive comprehensive support throughout their treatment.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker in our hospice center, you will primarily work with patients facing advanced illness, as well as their families. Your role will be fundamental in navigating the complexities of end-of-life care, offering emotional support, counseling, and guidance during a critical period. This unique client focus demands empathy, active listening, and a deep understanding of hospice principles, all aimed at improving patient quality of life.

Qualifications\n

• A Master's Degree in Social Work (MSW) or a related field is required.

• Must hold an active and current Pennsylvania State Licensure as an LCSW.

• At least two (2) years of experience in a mental health or clinical health setting is mandatory.

• Strong proficiency in evidence-based therapeutic modalities, including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), or Eye Movement Desensitization and Reprocessing (EMDR).

Preferred Experience\n
