RN - Case Management

Day shift, 5 days a week, with hours varying between 07:00 and 17:00. Job Description As a Registered Nurse Case Manager, you will be responsible for overseeing patient care and facilitating discharge planning in a Magnet-designated acute care hospital. You will work collaboratively with interdisciplinary teams to ensure that patients receive comprehensive care throughout their hospital stay and appropriate follow-up post-discharge. Responsibilities Coordinate patient care and discharge planning. Participate in multidisciplinary rounds and care coordination activities. Utilize electronic medical records (Epic) for documentation and communication. Ensure compliance with CMS and HIPAA regulations. Assess patient needs and evaluate care plans. Required Experience / Certifications / Licensure Basic Life Support (BLS) certification required. Acute care nursing experience required.
