Key Responsibilities\n

• Act as a primary liaison between clients, their families, and outpatient service providers to ensure a seamless transition of care.

• Engage thoughtfully with insurance companies to manage pre-certification processes, demonstrating a clear understanding of utilization management protocols.

• Participate actively in discharge and aftercare planning, tailoring strategies to client-specific needs and ensuring adequate follow-up support.

• Provide therapeutic meal support, instilling mindfulness and modeling a healthy relationship with food during sessions.
\n
• Deliver empathetic and affirming care that acknowledges and respects each client's diverse backgrounds and intersecting identities.

• Implement evidence-based practices and therapeutic techniques to enhance mental health outcomes for clients where appropriate.

• Document client interactions accurately and promptly within our electronic health record system, maintaining confidentiality and compliance with HIPAA regulations.

This role is specifically designed to support a diverse clientele, with a significant focus on older adults residing in hospice care. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Possession of a valid Licensed Clinical Social Worker (LCSW) license in the state of Maryland.

Compensation and Benefits\n
