Key Responsibilities\n

• Liaison Role: Act as a primary point of contact between families and outpatient providers, ensuring seamless communication and collaboration for effective patient care.

• Insurance Coordination: Interact with insurance companies to facilitate pre-certification and manage utilization, ensuring clients receive the benefits they require.
\n
• Discharge Planning: Actively participate in discharge and aftercare planning discussions to ensure a smooth transition for clients returning home or entering other levels of care.
\n
• Therapeutic Support: Provide therapeutic meal support while demonstrating and promoting healthy relationships with food, crucial for clients facing such challenges.

• Client-Centric Care: Offer safe, empathetic, and affirming care tailored to each client’s unique circumstances and intersecting identities, helping them navigate their emotional challenges.

Patient Population / Client Focus\n

In this role, you will primarily be working with older adults who are facing complex emotional and psychological challenges associated with hospice care. Your expertise will be vital in helping them and their families understand and cope with the intricacies of end-of-life transitions, ensuring dignity and respect throughout the process.

Qualifications\n

• Education: A Master's degree in Social Work from an accredited institution is mandatory for this role.

• Licensure: A valid LCSW license in the state of Maryland is required.

• Experience: Previous experience working with older adults and an understanding of the unique challenges they face in hospice settings are essential.
