Work Environment\n

The role is primarily remote, offering flexibility while being part of a dedicated hospice center team focused on comprehensive care for infants and toddlers. As you interact with children and their families from various backgrounds, your remote work will involve technology-based assessments and interventions, with occasional in-person meetings or visits to ensure thorough support. Key Responsibilities\n

• Conduct detailed evaluations and assessments of children in early intervention programs to determine developmental needs.

• Collaborate with families to develop and implement Individualized Family Service Plans (IFSP), tailoring strategies to their unique situations.
\n
• Monitor the progress and outcomes of implemented strategies, ensuring that each child receives appropriate support.
\n
• Provide evidence-based resources, skills training, and emotional support to families, empowering them to enhance their child’s development.
\n
• Utilize the coaching model and natural environment teaching methods to promote effective learning experiences.
\n
• Remain informed of and compliant with all applicable federal and state regulations to maintain the highest standards of care.

• Establish rapport and foster trust with children, families, and multidisciplinary team members to create a collaborative and positive environment.

Patient Population / Client Focus\n

The primary focus of this role is on young children under the age of three, including those with disabilities and developmental delays. Your work will directly impact these children’s lives by facilitating their physical, cognitive, social/emotional, and communication development. By partnering with families, you will help build their confidence in addressing the unique challenges they face and assist in navigating available resources.

Qualifications\n

• Must possess a Master's Degree or Doctorate in Social Work and be licensed by the Department of Public Health.

• Current Licensed Clinical Social Worker (LCSW) certification is mandatory.

• Valid driver's license for potential travel for in-person assessments or collaborations.
