Overview CP Rochester and Happiness House are private nonprofit health, educational, residential and human service organizations serving the Finger Lakes Region and Greater Rochester Area. We offer educational, therapeutic, recreational, residential and family support programs and services for children and adults with and without disabilities. The agencies serve 3,650 families each year and employ approximately 600 people. Job Title and Details Job Title: Physical Therapist Location: Winton Road Site, Rochester, NY 14623 Salary: $31.50 - $38.50 per hour, commensurate with experience Hours: Part-time, 20.0 - 29.9 hours per week. Clinic hours: Monday – Thursday 8:00am – 5:30pm; Friday 8:00am – 4:00pm. Signing Bonus: Eligible for a $3,000 signing bonus (50% after 1 year; 50% after 2 years). Travel/Other: Travel within local partner agencies as needed; some travel may be required. Requirements Bachelor’s degree in Applied Science and completion of an accredited Physical Therapy program. License or eligibility for a Physical Therapist license in New York State. Current New York State registration as a Physical Therapist. Valid New York State driver’s license. Experience working with individuals with developmental disabilities preferred. Proficient computer skills with knowledge of Microsoft Office programs. Demonstrated commitment to performing duties at a high level of performance. Summary/Objective CP Rochester/Happiness House exists to support people with physical and developmental disabilities in choosing and accomplishing individualized life goals. We provide services to people of all ages in the Finger Lakes and surrounding greater Rochester area, helping ensure they safely determine their own pathways and live as full members of their communities. Essential Functions Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources. Maintains confidentiality of information in accordance with agency policy. Upholds NYS Education Law/Practice Act as related to the practice of physical therapy. Performs evaluations to determine the need for physical therapy services. Ensures valid prescriptions are written for all individuals on caseload. Carries out treatment plans in clinic settings for Article 16 and Article 28 outpatient clinics, Preschool or School Programs as needed. Consults with teachers, habilitation staff and interdisciplinary team members to facilitate coordinated services. Collaborates with guardians, house staff or parents as integral team members in clinic, day program or school settings as needed. Communicates with physicians and other medical personnel to facilitate evaluation and treatment. Completes all paperwork in accordance with agency, program and department policies. Maintains knowledge of paperwork, Electronic Health Record and applicable Medicare/Medicaid regulations to provide appropriate documentation. Attends team, discipline and staff meetings as designated. Maintains a safe and orderly treatment area and equipment. Advises Supervisor of the need for replacement of old or ordering of new equipment. Meets with interdisciplinary team and Program Supervisor to review status and program modifications as needed. Participates in conferences, workshops and in-service training to enhance skills. Adheres to agency policies and procedures and department guidelines. Physical Demands The physical demands include frequent talking/hearing, sitting, standing, walking, using hands, and reaching. Lifting up to 10 pounds regularly; up to 50 pounds occasionally. Ability to move wheelchairs up to 250 – 300 pounds as needed.
